Open Rule Maintenance Application is disabled

I'm following the tutorial and all was good until I got to "Module 3 - Generating an RMA". I followed the steps to this tutorial but the RMA didn't open up. So, then I followed the steps for "Relaunching the RMA" but the menu option of (Tools-->Rule Maintenance Applications-->Open Rule Maintenance Application) is disabled. And when I hosted it manually in IIS and browsing to http://localhost:80/FICO_RMA


I get a log-on screen (see attachment) with no sign in button. I used different browsers and it seems only Chrome is displaying the page with my IIS Application Pool having .NET Framework 4 selected.


So, please help with either approaches to get the RMA up and running.
